All visa applicants who are former Chinese citizens are kindly requested to provide the following credentials when submitting the visa application:

I. For those who apply for a visa to China for the first time, please show your former Chinese passport and provide a photo copy of the Chinese passport; For those who have been issued a Chinese visa before, please show the passport onto which that Chinese visa is attached.

II. Please use Chinese characters to fill in your name (including your previous name if any) and birth place on the application form.
